Travel Health
Our practice nurses are available by appointment to offer medical advice regarding travel and vaccinations where appropriate. Please telephone reception to book an appointment.

If you are planning to travel abroad and require vaccinations, you will need to fill in the 'Travel Risk Assessment Form' below and return it to the surgery. You need to do this at least one month before you travel.
It can be sent via email to capccg.generalqemp@nhs.net
This will be passed to one of our Practice Nurses who will assess which vaccinations you will need and the length of appointment required. You will then need to call the surgery to make an appointment - Please allow at least a week between returning the form and calling to make an appointment.
Our receptionists will not be able to make you an appointment until your form has been assessed by the nurse. Travel Clinics are held in the practice on Tuesday afternoons.
Travel Vaccination Prices
PLEASE NOTE: We will only administer those vaccinations available on the NHS. Should you require any additional non-NHS vaccinations (indicated with a * on the final page of the form), you will need to have these done at a Private Travel Clinic.
